Your New Patient Experience
We are so excited to have you join our family at Main Line Dental Health & Wellness. Comprehensive care is our specialty, and we look forward to having you experience it firsthand. At your new patient visit, we update a wide variety of diagnostics and metrics, so please allocate 90 to 120 minutes for your new patient visit, especially if you have not completed the paperwork below beforehand.
We obtain a full set of dental radiographs, unless you provide us with reports of radiographs taken within a year, at least two days before your visit. We also obtain a 3-D image to evaluate your bite, a full set of dental photographs, a comprehensive periodontal evaluation, and a salivary diagnostic to assess your pathogen levels. Fees are incurred for our dental radiographs; the only other cost to you is a lab fee for the salivary diagnostic.
We firmly believe in comprehensive dentistry that helps not only your oral health, but your total body health, so all other diagnostics are complimentary. By filling out these forms before your visit with our friendly dentist in Bryn Mawr, PA, and the Main Line, you can save some time during your visit to our office. In fact, just completing your new patient forms in advance can cut down your appointment time by 15-30 minutes! Simply fill out the forms on our website, submit them to us online before your appointment, or bring a printed version when you arrive for your appointment at Main Line Dental Health & Wellness. Financial Options
Main Line Dental Health & Wellness offers many financial options to help you and your family receive the affordable, high-quality dental care you deserve. We are out of contract with all insurance companies as we believe we do not want insurance to limit or dictate how we treat our patients. We submit claims to all PPO insurance companies as an out-of-contract provider to ensure you get the insurance benefits you deserve. CareCredit and Lending Point offer convenient and affordable monthly financing plans designed to fit your unique needs and budget. You can apply for CareCredit and Lending Point online or at our office with the help of one of our friendly staff members.

Quality Dental Plan
We understand that not all our patients have access to dental insurance or/and your dental insurance limits your care. If you are uninsured or choose not to utilize your dental insurance, you can join our Quality Dental Plan membership. This membership plan is designed to give you access to the highest quality dental care we can provide at a reasonable cost. It also allows you to receive discounts with no annual maximum or deductibles, unlike traditional dental insurance, regardless of how much treatment or products you receive. You receive discounts on services such as Invisalign, Botox, tooth whitening options like ZOOM and KOR, preventative therapies, and dental products purchased from the office, something your dental insurance can’t or doesn’t provide. Ask one of our expert administrative team members about our Quality Dental Plan today. We have options for your hygiene visits two, three, or four times a year for preventative and periodontal patients. This keeps your hygiene care affordable and reduces the cost of treatments.
